  • Link for Child Abuse Form: 
    PA Child Abuse History Certification

    • Employee: fee of $13.00 (see “General” on instructions page) 

    • Print and mail these forms – online submissions have fees

  • Link for Criminal History Background Check:
    PA Criminal History Background Check

    • Volunteers, click on the link for the SP 4-164A form

    • Employees: click on link for the SO 4 164 form

      • Employee: fee of $22.00

      • Employees: Submit your receipt to Phyllis Sanford and the Church will reimburse you.

Results take approximately 10 days to 2 weeks.

Identogo Hyperlink and Identogo Registration Process

Process to obtain Live-Scan Digital Fingerprinting in compliance with PA Department of Human Services

  • Our insurance requires us to comply with the PA Department of Human Services (DHS) ACT 153. DHS is partnered with Identogo to obtain the Digital live-scan prints to ensure they meet all requirements. 

  • Unfortunately, the Identogo location that we previously had right here in Willow Street is now closed.  The closest location to our church is now in New Holland, PA. First, you will need to go on to https://www.identogo.com/ and fill out your information to obtain a date and time. There Is also an Identogo location in Elizabethtown, PA.

    • If you prefer to go to Elizabethtown, use the same link and request Elizabethtown location when completing the form.

  • When asked to enter a  Service Code, enter:

    • Volunteer is 1KG6ZJ     

    • Employee is IKG 756

  • The information for the New Holland location is:
    Identogo (Enrollment Center is in the Weaver Group Services, LLC)

    112 E. Main Street (Note: the Identogo Website list the address as 116 – that it is incorrect.) New Holland, PA

    • Phone: 717-354-4361 

    • Phil is the name of the fellow who does the fingerprinting. His Son, Wade, sometimes answers as he works in the adjacent Chamber of Commerce.  The cost should be approximately $22.95 for volunteers and $24.95 for Employees. Save your receipt – Return it to Phylis Sanford for submission and the Church will reimburse you.

    • Parking is in the back. 

      • Wade said the best street marker is to look for PJ's Steak and Hoagie Restaurant - easy to spot. Turn in there, parking for Identogo is back there.
